When you submit your STR, it will be entered into our support database for tracking purposes. After an initial assessment by our support department, a customer tracking number and support technician will be assigned. Please be as specific as possible when describing the problem. 

You may print the form below and fax it to Perii at 201.802.1085 or E- mail it and attach the your problem code to servicerequest@perii.com along with the information requested
